| | | | | | | | Current Situation | H-GAC issued a request for proposals in January 2017 to solicit providers of evidence-based disease intervention education. This service includes seminars and classes to help participants manage and control chronic disease conditions and improve their quality of life. Topics include Diabetes Self-Management, Chronic Disease Self-Management, Hypertension Self-Management, A Matter of Balance, and Stress Management for Caregivers. Classes meet two to three times per week for six to eight weeks.
We received two proposals for evidence-based training;both from prior year contractors who have performed satisfactorily for us.
The Aging Programs Advisory Committee recommends approving contracts with each of these providers through September 2017 as follows:
