Background

H-GAC as the Area Agency on Aging annually contracts with local providers to offer a variety of services for older individuals living in Austin, Brazoria, Chambers, Colorado, Fort Bend, Galveston, Liberty, Matagorda, Montgomery, Walker, Waller and Wharton counties.

Current Situation

H-GAC issued a request for proposals in January 2017 to solicit providers of evidence-based disease intervention education.  This service includes seminars and classes to help participants manage and control chronic disease conditions and improve their quality of life. Topics include Diabetes Self-Management, Chronic Disease Self-Management, Hypertension Self-Management, A Matter of Balance, and Stress Management for Caregivers.  Classes meet two to three times per week for six to eight weeks.

We received two proposals for evidence-based training;both from prior year contractors who have performed satisfactorily for us.
